ABSTRACT
Objective To investigate the clinical effects of Shenlian capsule,trastuzumab combined with SOX regimen in the treatment for HER-2 positive advanced gastric carcinoma.Methods A total of 314 patients with HER-2 positive advanced gastric carcinoma in our hospital were randomly divided observation group and control group,157 cases in each group.The observation group was treated by SOX regimen and trastuzumab,while the control group was given Shenlian capsule on the base of control group.After treatment,the short-term effects in the 2 groups were evaluated.Before and after the treatment,the levels of SDF-1,CXCR4,FSP-1 and MRP-l4 in the 2 groups were detected.During the treatment,the rates of neurotoxicity,gastrointestinal reaction,liver function injury,renal function injury,rashes and bone marrow suppression were observed in the 2 groups.After the treatment,3 years follow-up was performed,the 1 year survival rate,3 year survival rate,overall survival (OS) and progression-free survival (PFS) were observed.Results After the treatment,the control rate (92.7％ vs.79.9％;Z=3.812,P=0.037) and efficiency rate (78.1％ vs.66.4％;Z=3.712,P=0.039) were significantly higher in treatment group than which in the control group (P＜0.05).After the treatment,the levels of SDF-1,CXCR4,FSP-1,MRP-14 in treatment group were significantly lower than control group (t=4.091,4.310,3.972,3.821,P＜0.05).During the treatment,the rates of neurotoxicity,gastrointestinal reaction,liver function injury,renal function injury,rashes,bone marrow suppression were significantly lower in treatment group than control group (x2=5.182,4.671,4.491,5.091,5.204,4.925,P＜0.05 or P＜0.01).After the treatment,the PFS,OS (t=3.714,3.612,P＜0.05),3 year survival rate (x2=3.105,P=0.043) were significantly higher in treatment group than the control group.Conclusions The Shenlian capsule,trastuzumab combined with SOX regimen has a good effect and low toxic effects in the treatment of HER-2 positive advanced gastric carcinoma,and they could prolong the survival time.

Purpose To detect the HER2 gene amplification by fluorescence in-situ hybridization(FISH) and to explore its clinicopathologic relationship with the breast cancer.Methods A prospective study was conducted in 50 cases of breast invasive ductal carcinoma from Beijing Toren Hospital. Clinicopathologic data were summarized and FISH was performed in the paraffin-embedded sections for HER2 gene amplificayion using DNA probe, and immunohistochemical stain for ER, PR and HER2.Results The average age of the patients was 55.5 years. The pathologic grading showed that 11 cases were in grade Ⅰ, 30 cases in grade Ⅱ, and 9 cases in grade Ⅲ. The TNM staging showed that 13 cases were in stage Ⅰ,15 cases in ⅡA,13 cases in ⅡB,6 cases in ⅢA,2 cases in ⅢB,and 1 case in ⅢC.The median metastasis rate of lymph node was 6.91%.33 cases were positive for ER,and 32 cases positive for PR.For HER2 detection, 40 cases were positive by IHC and 33 positive by FISH. HER2 gene amplification by FISH was closely related with the expression of HER2 protein by immunohistochemistry, but not significantly related with pathologic grading, The TNM staging, median lymph node metastasis rate, ER and PR status (P>0.05). FISH test was positive in 3 cases of tumor embolus and 3 cases of multiple primary tumors. Conclusions FISH and immunohistochemistry for detecting HER2 have a good conformance, HER2 gene amplification may be related with tumor embolus and multiple primary tumors, but it can not be used as an indirect marker to predict the prognosis.

Objective To investigate the relationship between vascular cognitive impairment(VCI) and the angiotensinogen(AGT) gene ( G-6A and M235T) polymorphism. Methods Randomnized controled study was ap- plied in the study. AGT gene G-6A and M235T genotypes of 67 cases with VCI and 71 normal controls were deter- mined by polymerase chain reaction (PCR). Results For the location of M235T, the frequencies of T allele(0.73 I and TT genotype ( 0.52 ) were observed in VCI compared with control group ( 0.68,0.45, P > 0.05 ). The odds ratio associated with TT/MM genotype was 0.544 ( 95% CI 0.208～1. 424 ,P > 0.05 ). For the location of G-6A ,the fre- quencies of A allele(0.69) and AA genotype (0.48) were observed in VCI compared with control subjects (0.63, 0.39,P > 0.05). The odds ratio associated with AA/GG genotype was 0.602 ( 95% CI 0.252～1. 738, P > 0.05 ). There was no difference in allele distribution between 67 VCI patients and the controls. Conclusion There is no correlation between vascular cognitive impairment and AGT gene polymorphisms of M235T and G-6A. AGT gene pol- ymorphism is not included in the risk factors for development vascular cognitive impairment.

OBJECTIVE To observe the effects of Dexamethasone on the expression of cyclic nucleotide-gated channels (CNG channels) mRNA of olfactory receptor neurons (ORNs) by real-time quantitative reverse transcription-polymerase chain reaction(RT-PCR). METHODS Forty Wistar rats were randomly divided into four groups: 24-hours Dexamethasone treated group and its control group; 2-weeks Dexamethasone treated group and its control group. Dexamethasone was injected i.p. (1 mg/kg for 24-hours group, 0.2 mg/d for 2-weeks group). Control group rats were injected with the same volume of normal saline.
